The CBC Portal and the DPS Fingerprint Card Portal (also known as the AZDPS Public Services Portal) look very similar and can both be accessed using the same username and password. Be sure to navigate to the correct portal during each step.
The correct portal can be recognized by the following logo:
CORRECT
If you are on the DPS portal, you will see the following logo:
This indicates you are on the wrong portal, and will need to navigate back to the CBC portal HERE.
INCORRECT
To proceed, follow either Route A or B based on your situation:
A) If you already have a DPS account, select the login hyperlink (located within the statement above the 4 boxes on the CBC homepage) and use your username and password to proceed with the background check process.
🚨NOTE: If the system prompts you for a fingerprint card "code," you are on the wrong portal and will need to navigate back to the CBC portal.
B) If you DO NOT already have a DPS account, create one by selecting the "Individual/Personal" account option—do not choose the "Caregiver" account option. (The term "Caregiver" on the CBC site refers to foster caregivers, which is different from your role as a caregiver with AFC.)
When setting up a new CBC account, you may be prompted to link your Fingerprint Clearance Card. If so, you'll need to connect your card or application to your DPS PSP account. To do this, visit psp.azdps.gov🔗 and follow these steps:
Click ‘Services’ in the header.
Click the ‘Fingerprint Clearance Card’ tile.
For ‘What action do you need to take?’, select ‘Apply for a card / Request a replacement’ and click ‘Continue’.
For ‘Have you applied for a DPS Fingerprint Clearance Card in the past?’, click ‘Yes’.
Enter your FCC Application Number or Card Number and click ‘Continue’.
This request DOES NOT require a notarized affidavit
When submitting this request, you will need the following details for AFC Systems:
CBC Account Email: afcsystemsllc@gmail.com
DES Division Name: DDD and/or HCBS
Provider/Vendor ID: RFQVA DDD-2024
Follow the instructions and complete all the necessary sections
This request DOES require a notarized affidavit. You can access a (.pdf) copy of the affidavit HERE.
Complete the affidavit and sign it ONLY in the presence of a notary, who will then notarize it on page 1. Page 2 can be left blank. DO NOT sign the affidavit beforehand—wait until the notary is available to witness your signature.
Follow the instructions and complete all the necessary sections, then submit your request.
